The one idea that makes this work. Some of what gets said standing in a venue is a decision and some of it is thinking out loud, and the notes do not mark which is which. Keeping those two apart is the whole job. A line that reads like a decision becomes a purchase order or a truck booking, and by the time anybody notices it was only an option floated near the dock, the deposit is gone. So every line goes into one of four piles: decided, owned, open, or needs confirming. When you cannot tell which, it goes in needs confirming, which is always safe and never a failure.
